Vellum. 34.5–37.5 x 23–28 cms (pp. 3–4, 31–36 x 21.5–23 cms; pp. 7–8, 32–34 x 21.5–26.5 cms; pp. 9–10, 30.5–32 x 21.5–26 cms; pp. 37–8, 31–31.5 x 15.5–26.5 cms; pp. 43–4, 29.5–30.5 x 26 cms; pp. 53–4, 30.5–35 x 25–27.5 cms; pp. 57–8, 31.5–36.5 x 25–25.5 cms). Pp. 70, paginated (earlier partial pagination in ink supplemented by modern pencil pagination) [1]–[70], with pp. [1]–[3], [13], [17] and [70] unnumbered; pencilled pagination 38–41 corrected from 39–42. No scribal signature. Scribes: (1) main scribe, pp. 1–10b6, 10b20–47b9, 48a1–bz; (2) pp. 49a1–52b36, 52b45–54a22, 54a28–70bz; (3) pp. 10b7–19; (4) pp. 47b10–z; (5) pp. 52b37–45, 54a22–7.

Housed in modern conservation box labelled `TCD / MS / 1321' on spine and stamped `TCD ms 1321' inside spine. Bound in blue half-rexine, cardboard sides covered in blue cloth; cloth partly loose on back cover. `medical / treatises' and `H. / 3.2.' in gold on upper and lower spine, respectively; `1321' typed on piece of paper affixed to base of spine and covered with cellulose tape. Four endpapers front and back, with third and fourth front, and first and second back, of slightly smaller dimensions; pastedowns, recto of first front endpaper and verso of fourth back endpaper marbled. `Bound Feb 1898' (in pencil) on verso of first front endpaper; `H. 3. 2. / (1321)' (in pencil) on recto of second front endpaper. Printed label with three required categories affixed to p. 1, upper margin, with entries supplied in ink: `Class. H / Tab. 3 / No. 2'. Fragment of label bearing description of contents, transversely: `(...) on Botany, (.)[...] / [...]es in Vellum. / [H. 3. 2 in later ink]', p. 70, inner margin; above this, to the left, fragment of same label, transversely: `Medic[...]', and below, further label bearing number: `H / 3 [? .2]'.

Collation: iiii8, iv10 + 1 (wants 8 (stub visible after p. 62) with no loss of text; wants 10 (stub visible after p. 64) with loss of text; pp. 65–6 inserted between 10 and 11); loss after p. 48. Signatures `b', `c' and `d' on lower margins of pp. 17, 16 and 48, respectively, indicate that pp. 1–16 are misplaced, belonging after pp. 17–32; textual sequence shows order of leaves in pp. 1–48 to be as follows: pp. 17–32, 1–16, 33–48; blackening and heavy staining on p. 1, consonant with prolonged use as outer cover, shows that misplacement precedes binding. Double columns, with single inner bounding lines, with double outer bounding line for inner column, and single for outer column, 51–73 lines, ruled in brown; grid distinct p. 29. Pricking for double columns with single inner and outer bounding lines visible in upper and lower margins towards leaf-edge, and for top and bottom lines in inner and outer margins; pricking for horizontal text-lines visible in inner and outer margins of pp. 55–6, its conjunct pp. 63–4, also in singletons pp. 57–8 and 65–6. First line(s) of each chapter-incipit (first word only, p. 50b46) in display script, also opening word(s) of some subsections of text; spaces reserved for ornamental opening initials (5- to 1-line) in chapter-incipits (except at p. 50b46), but initials nowhere filled in; space otiose p. 21b23 as first letter of incipit written; reserved spaces without guide-letters in section of ms written by main scribe (pp. 1–48), with guide-letters in section written by second scribe (pp. 49–[70]); initial crudely written by later hand in reserved space, p. 16a. No catchwords, but last word on page occasionally repeated at beginning of following page in section of manuscript written by main scribe. Textual corrections or omissions in outer margins of pp. 6, 15, 32 and 44, and in inner margins of pp. 18 and 50; marginal heading in outer margin of p. 49. Occasional pencil marks by later reader (?), e.g. pp. 34 (outer margin), 43 (inner margin), 62 (between cols). Vellum defective pp. 5–6, 9–12, 15–16, 21–4, 27–8, 31–2, 35–6, 41–2, 49–50, 51–2, 55–8, 63–6. Diagonal cut in pp. 51–2 repaired prior to writing with bright red thread. Piece cut out from pp. 43–4, from lower margin into text area, with loss of text on pp. 43aw–z, b35–8, 44a38–40 and bw–z, and with loss of final lines of text on pp. 43b and 44a. Slight hole in pp. 11–12 with no loss of text; holes with slight loss of text in pp. 65–8, and with more extensive loss in pp. 1–2, 69–70; upper inner corners of pp. 63–4, and particularly pp. 65–8, decayed with loss of text in opening lines of pp. 65a and 67a, and of final words in opening lines of pp. 66b and 68b. Manuscript extensively repaired in 19th century, with repairs to damaged areas occasionally obscuring adjacent text, particularly at p. 67a1–31. Spines of the following bifolia reinforced full length from outside by tape, sometimes overlapping onto inner spine: pp. 1–2/15–16, 17–18/31–2, 49–50/69–70, 51–2/67–8, 53–4/stub following p. 64; spine of pp. 57–8/stub following p. 62 similarly reinforced from inside with overlap at top onto outer spine; spines of the following bifolia reinforced at top and/or bottom only: pp. 33–4/47–8, 35–6/45–6, 37–8/43–4, 39–40/41–2, 55–6/63–4, 59–60/61–2; singleton pp. 65–6 attached full length by tape to p. 67; tape repairs or reinforcement to outer edges of pp. 1–2, 11–12, 17–24, 31–2, 35–6, and also to p. 11, lower margin, and p. 21, outer margin. Some staining throughout, particularly at edges and on pp. 48–9; occasional illegibility due to staining, pp. 3, 8, 49 and 53; heavier staining, with consequent illegibility, on pp. 2, 65, 68–9; pp. 1 and 70 blackened and almost entirely illegible.
