15th or 16th cent. Vellum. 29–30 × 20–21 cm (f. 6, 29.5–30 × 18–20.5). Ff. 7, numbered in pencil. No scribal signatures. Scribes: (1) ff. 1–2, 3ra9–bz, 3va19–bz, 4ra9–24, 4ra35–b38, 4rb40–va37, 4vb6–5ra12; (2) ff. 3ra1–9, 3va1–19, 4ra1–8, 25–34, 4rb38–9, 4va38–b5, 5ra13–26, 5rb14–27; (3) ff. 5ra27–b14, 5rb27–6rb22; (4) f. 6v1–z; (5) f. 7r1–z. Unbound, with leaves now loose; housed in paper folder marked ‘1310’ (in pencil) on front cover. Four sets of stab-holes; three vellum cut-outs, probably connected with an earlier binding, visible between ff. 4 and 5 as foll.: (a) strip (0.5–1.5 × 1–1.5 cm) with two tabs, one above, one below, projecting from its inner edge; letter ‘a’ in Gothic script on recto with double horizontal rule in red below; letters ‘[? duc]’ (with portions of first and third letters cut away) in Gothic script on verso with single horizontal rule in red below; (b) strip (7.5–8 × 2.5–4 cm) with smooth inner edge and with outer edge shaped into six castellated projections; traces of erased writing on both recto and verso; (c) blank scrap (1.5 × 1.5) with single tab projecting from its centre at inner edge; tabs of strips (a) and (c) fitted into stab-holes at top and bottom, respectively, in spine of bifolium 5/6 (ff. 4–5), with tabs visible in opening of ff. 3v and 4r and again in opening of ff. 5v and 6r*; strip (b) now loose, probably formerly attached to second and third set of stab-holes in spine; a vellum thong (0.25 × 3 cm), a further remnant of earlier binding, passes through second set of stitch-holes from top in spine of bifolium 4/7 (ff. 3, 6), with its main part now protruding into opening of ff. 2v and 3r (imprint of thong on f. 3r, inner margin), and its tail protruding into opening of ff. 5v and 6r. Number ‘H. 2. 12 / No. XI’ written over beginning of first and second lines of text on f. 1rb; number ‘1310’ (in pencil), f. 1r, centre of upper margin.

Collation: i10 wants 3, 8 and 9: loss between ff. 2/3 and 6/7. Double columns in ff. 1–6r, ruled in dry point and in brown, 40–52 lines; single column on ff. 6v (31 lines), 7r (36 lines) and probably 7v (mostly illegible); remains of pricking for double columns visible in upper and lower margins of most leaves (including f. 7), and for horizontal text-lines in inner and outer margins of all leaves; chapter-incipits on ff. 1r, 3r, 5, and 6 in display script; ornamental initials in text on f. 6v; space reserved, marked with guide-letter ‘u’, for 5-line opening initial at f. 6v1, but corresponding initial not written. Initials, incipits and capital letters frequently rubricated in ff. 1v–3r, also various words and phrases within texts, in particular Latin words and citations, titles, proper names, numbers, ceann fo eite and closing sign ‘⁊rl’; traces of red colouring visible on f. 1r; final ten lines of text on f. 2rb extensively rubricated, with chapter-incipit on f. 3ra3–5 entirely coloured in red; rubrication discontinued after f. 3rb31; slight tinting on inner and outer margins of f. 3 perhaps transferred from rubrication on leaf lost between ff. 2 and 3. Crudely decorative interlace border in black ink added later in outer edge of f. 2r. Vellum defective in ff. 2, 4 and 7; defect in inner lower margin of f. 4 repaired with red thread; cut in outer edge of f. 6 repaired with green thread; slight tear in outer edge of f. 7. Staining throughout; front and back pages heavily stained, scratched and rubbed, indicating their use as outer covers, with front page largely illegible, and back page almost entirely illegible; heavy localised stains penetrating from upper to lower leaves, ff. 1–5r.

7r. [Regimen sanitatis Salernitanum (Latin only).] Acephalous. Beg. here Si tibhi defichiant medichi [medici above line with caret mark] tibi fian<t> hec tria mens leta requies / moderata dieta. F. 7r ends Saluia sal <uinum> pibar allea petrosilinum ex his fit salsa nisi sit <comixtio>. Text apparently continues on f. 7v which is mostly illegible.
